Nicotinamide adenine dinucleotide, or NAD+, is a coenzyme involved in cellular energy metabolism, redox reactions and enzyme activity. These biological roles have generated interest in aging, neurological health and mental health. They do not, however, establish intravenous NAD+ as a treatment for depression, anxiety or another psychiatric condition.

Why Researchers Study NAD+ Biology

Brain cells require substantial energy and depend on healthy metabolic pathways. NAD+ also participates in processes related to DNA repair and cellular stress responses. Researchers are studying NAD+ metabolism and several precursor compounds, but findings from cells, animals, oral supplements and intravenous products should not be treated as equivalent.

What The Clinical Evidence Can Support

Human evidence for commercially administered intravenous NAD+ remains limited. A recent retrospective pilot compared IV NAD+ with intravenous nicotinamide riboside in a small real-world sample. It reported short-term tolerability findings, not proof that NAD+ treats depression or improves long-term mental health.

Small studies can help identify research questions, but they cannot establish broad effectiveness. Patients should be cautious with claims that an infusion will “reset” the brain, cure addiction, reverse depression or guarantee mental clarity.

Mental Health Assessment Comes First

Low mood, anxiety, poor concentration, sleep disturbance and fatigue can have many causes. A qualified clinician may assess symptom duration and severity, medication, substance use, sleep, thyroid function, anaemia, nutritional deficiencies and other medical factors. Urgent help is needed when there is risk of self-harm, psychosis, severe functional decline or another crisis.

Evidence-based mental healthcare may include psychotherapy, medication, lifestyle support and treatment of contributing medical conditions. NAD+ discussions should not delay, replace or interrupt appropriate psychiatric or psychological care.

Safety Questions For IV NAD+

  • What exact ingredient, dose and infusion rate are proposed?
  • Why is IV administration being considered rather than another approach?
  • What evidence supports the stated goal?
  • What symptoms can occur during infusion and how are they monitored?
  • Could medication, pregnancy, kidney, liver or cardiovascular issues affect suitability?
  • Who supervises the infusion and manages an adverse reaction?

Published preliminary data have described gastrointestinal symptoms, increased heart rate and chest pressure during some NAD+ infusions. Individual assessment and monitoring therefore matter, even when a service is marketed as wellness support.

A Responsible Role In Care Planning

NAD+ may be discussed as an adjunctive wellness or metabolic-support question after standard assessment, not as a stand-alone mental health solution. The consultation should define the goal, explain uncertainty, review contraindications and decide how any response would be measured.

The most credible answer is sometimes that evidence is not yet strong enough for the promised outcome. Patients benefit when emerging biology is presented with precision rather than converted into certainty.
